vendor must be prepared to provide our Vendor Management Department with a variety
of documents. The list includes:
- Copies of Diplomas/Certificates recieved and Designations held. (This is only necessary
for induviduals, not Corporations)
- Copies of State License or Certification.
- Copy of Errors and Omissions declaration page.
- Copy of resume or Business Pamphlet.
- Copy of a W-9 Form.
